What is Trinity Community Care

Trinity Community Care is a faith-based clinic that provides free medical and dental care to those who cannot afford it, delivered with unconditional Christian love. We also offer social and spiritual support services to help connect patients to overall health and wellness.

Why a Free Clinic?

Our faith calls us to compassionately care for those who needs us. Trinity Community Care provides an opportunity for us to serve the members of our community in need of medical and dental care, giving them support to lead healthy lives.

Who does Trinity Community Care Serve?

Services are provided for adults that meet the following criteria:

How do I get Services from Trinity Community Care

Eligibility must be established prior to receiving services at the clinic. Eligibility interviews are done by appointment. To schedule your interview, please call us at 586-649-3014. Once your eligibility has been certified, we will schedule your first medical and/or dental appointment.

How is Trinity Community Care Funded?

Trinity Community Care is not a government program. We are self-funded and rely on grants and gifts from individuals, businesses, and other groups to fund our mission.

What are the hours of the clinic?

Trinity Community Care is run by volunteers. Appointments and clinic sessions are based on volunteer availability.

Where Is Trinity Community Care?

*The clinic is on the west side of Van Dyke, about ½ mile north of 21 Mile Road.

We are located in the back building of the Shaggy Dog Plaza, just north of the McDonald’s.

Laboratory and imaging services are provided by Beaumont Health System. In some instances, it may be necessary to go to one of their nearby locations for a particular test.

Patient Information

Criteria for Patients:

Patients are required to sign a Patient Consent Contract for treatment by volunteers.

Patients must have ID and provide documentation supporting their eligibility for services.

 

Services Offered:

Medical Services provided are similar to those offered by a family practice or general care practitioner:

Dental Services provides individualized care such as:

 

Support Services include resource assistance, prescription assistance, education and spiritual counseling.

Referrals and other resources may be available upon request for additional services.

No controlled or narcotic drugs will be prescribed or available.
